• Ridgeview Sports Participation Form - this form must be completed by your physician or Urgent Care facility. Tell your physician you need a "Sports Physical".  Email our program the signed CLEARANCE PAGE only(page 9 of the packet). Keep the paper copies & turn them in to Ridgeview Athletics Dept.( you will be required to have a physical to participate in other after-school activities).
